DUES/TRANSFER/MEMBERSHIP UPDATE FORM

Duesare $10.00 per year. Lodge dues run from 1 December of a current year through 30 November of the following year. Dues are payable at any time throughout the year but are considered delinquent if not paid by 15 December. There is no cost to transfer your membership provided your dues are current with your parent lodge. If your dues are not current there is an additional charge of $5.00. Unless your dues are current, you will no longer receive our newsletters. This form can also be used to update or change your mailing address, phone number or other membership information maintained by our Lodge.
